Perinatal Pelvic Health Services
Clinics are located across Humber and North Yorkshire in: Goole, Grimsby, Harrogate, Hull, Scarborough, Scunthorpe and York. Please only complete this form if you would like to receive care at one of these locations.
Please complete all sections of this referral form to the best of your ability. This will ensure it is triaged correctly, and your care is tailored to your pelvic health concern.
Please be aware, we are not an emergency service, therefore if you are experiencing signs and symptoms of urinary retention (*), urinary tract infection (**) or wound infection (***), please contact your maternity assessment unit, GP or 111 if out of hours.
If you are experiencing a loss of sensation when passing urine and/or to open your bowels, please call 111.
If you notice blood in your urine, please contact your GP.
* Signs of acute urinary retention include a sudden inability to pass urine, usually with some pain or discomfort in your bladder.
** Signs of urinary tract infection including pain or a burning sensation when urinating, urine that looks cloudy, darker or stronger smelling than usual, a need to pass urine more frequently/urgently
*** Signs of wound infection or breakdown including an increase in pain, new bleeding or pus-like discharge, or feeling unwell, itching, foul smelling discharge.
**** Signs of bleeding or increased post partum bleeding, please contact 111 or the maternity unit in your area.
If your symptoms change at any point, or you do not feel you have received the care you need, you can re-refer at any time OR you can speak to your Midwife, GP or Health Visitor.
